Design of a novel clinical trial of prehospital pediatric airway management

Emergency Medical Services personnel are often the first to intervene in the care of critically ill children. Airway management is a fundamental step in prehospital resuscitation, yet there is significant variation in current prehospital airway management practices. Our objective is to present a methodologic approach to determine the optimal strategy for prehospital pediatric airway management. We describe the conceptual premise for the Pediatric Prehospital Airway Resuscitation Trial... We developed an innovative design to enable comparison of the three predominant prehospital pediatric airway techniques (bag-mask-ventilation, supraglottic airway insertion, and endotracheal intubation) in…

Frequently Asked Questions about the Pedi-PART Trial

Patients in this study will have suffered a serious illness or injury causing difficulty breathing, therefore requiring immediate life-saving interventions. These types of emergencies occur unexpectedly, and it is not possible for people to sign up to take part ahead of time. Most patients will be unconscious (unable to speak or hear) and too sick to consent to immediate treatment or participation in the study. Patients will therefore be entered into the study prior to consent.

Pediatric Prehospital Airway Resuscitation Trial

This study is a Phase 3, multi-center, Bayesian Adaptive Sequential Platform Trial testing the effectiveness of different prehospital airway management strategies in the care of critically ill children. Emergency Medical Services (EMS) agencies affiliated with the Pediatric Emergency Care Applied Research Network (PECARN) will participate in the trial. The study interventions are strategies of prehospital airway management: \[BVM-only\], \[BVM followed by SGA\] and \[BVM followed by ETI\]. The primary outcome is 30-day ICU-free survival. The trial will be organized and executed in two successive stages. In Stage I of the trial, EMS personnel will alternate between two strategies: \[BVM-only\] or \[BVM followed by SGA\]. The \[winner of Stage I\] will advance to Stage II based upon results of Bayesian interim analyses. In Stage II of the trial, EMS personnel will alternate between \[BVM followed by ETI\] vs. \[Winner of Stage I\].

Dr Julie Leonard and Dr Henry Wang visit the studio as we explore Pedi-PART. This national research study examines advanced airway management for children in the prehospital setting. We detail the study and consider exception from informed consent.

The Pediatric Prehospital Airway Resuscitation Trial (Los Angeles, California)

In this study, we will determine which method works best for child survival by comparing the 3 methods that are used every day by emergency medical providers to manage airways and support breathing. Participating EMS agencies will be assigned to use a different method each day. This will allow researchers to compare the effectiveness of each method. If the assigned method is unsuccessful then EMS may rescue with any other airway method. EMS agencies from 10 different cities across the US will participate in the trial. These EMS agencies are collaborating with the Pediatric Emergency Care Applied Research Network.
