In general, cases that have historically been called funiculitis are really an inflammation of just the ductus deferens. Therefore it has been said that the latter term is preferable. Occasionally the inflammation may spread such that the entire spermatic cord is involved forming a true funiculitis. Symptoms of acute vasitis are relatively non-specific and may include a painful swelling in the groin, symptoms of a urinary tract infection, or a dull ache in the pelvis. Funiculitis, an inflammation of the spermatic cord, usually results from infection of adjacent structures like the epididymis, testis or the urethra. It is most frequently caused by a bacterial infection, but has a broad (non-infectious) differential diagnosis. Vasculitis should be considered in the differential diagnosis of a funiculitis that does not respond to antibiotic therapy. PET CT is an important tool for the diagnosis of vasculitis in patients with an atypical presentation and inconclusive diagnostic testing.
