What Does It Mean to Be Immunocompromised?
Experts often liken the immune system to an army that protects you from foreign enemies, like viruses and bacteria, as well as domestic ones, like cancer cells. But some people have an immune army that is not working as it should, because they were born that way or because of something that occurred later in life. Researchers have identified more than 430 so-called primary immunodeficiencies, rare conditions that are caused by genetic variants and weaken the immune system. Some of these can be detected through routine newborn screenings or other blood tests shortly after birth.

With rapidly improving genetic technology, the field of PIDs is expanding exponentially. As next-generation sequencing continues to discover new defects, there is a growing need to improve the functional immunological assays that uncover the underlying pathophysiology and molecular mechanisms. Gene therapy, newer biological agents, and possibly RNA interference (iRNA) therapies are on the horizon and promise to dramatically improve outcomes for individuals with PIDs and their families.

Immunodeficiencies in adults are increasingly recognized yet often remain underdiagnosed, leading to significant morbidity from recurrent infections, autoimmunity, and malignancy. Both primary immunodeficiencies (PIDs), now known as inborn errors of immunity (IEI), and secondary immunodeficiencies (SIDs) contribute to immune dysfunction in adults. Although SIDs are more common in adults due to factors like medications, malignancies, metabolic disorders, chronic conditions, and protein-losing conditions, IEI—particularly common variable immunodeficiency (CVID)—can also manifest in adulthood with diverse clinical features.

Severe Combined Immunodeficiency (SCID) may be best known from news stories and a movie in the 1980s about David, the Boy in the Bubble, who was born without a working immune system. Caused by defects in any of several possible genes, SCID makes those affected highly susceptible to life-threatening infections by viruses, bacteria and fungi.
What Living With A Challenged Immune System Taught Me About Happiness
So yes, I am sick. My immune system is challenged because of CVID, a primary immunodeficiency disorder that leaves me highly susceptible to recurrent infections and illness. Because of it, each month I receive a treatment that contains antibodies collected from the plasma that healthy people have donated.
